Gregorio Carafa continued the improvement of Malta's fortifications, including Fort Saint Angelo and Fort Saint Elmo.

His rule also saw Baroque renovation of the Auberge d'Italie in Valletta.
On Malta the Order developed hospitals, fortifications and a maritime presence while governing the islands. Its religious and charitable purposes continued alongside these public responsibilities.
